Our office engages in following up with patients to help remind, support and answer questions about your care. We follow up with whatever is needed to support your care from test results to reminders about outstanding orders made by your doctor.

We believe that health services should focus on what is best for our clients and their health as they are our true customer. We achieve this by setting a high standard of care and extending the traditional medicine offerings with services that support a whole health approach.
These memberships offer extended health services integrated direct with your primary care office. Joining means our organization supports you with the services offered under the membership of your choice as well as with our primary care physicians.
Medical visits are billed through your insurance. The membership fees cover the other services offered. See individual programs for details. The non-medical services within these memberships are not covered by insurance companies.
These memberships focus on preventing disease as well as treating it. To achieve this, we have established a multidisciplinary team of preventive health professionals and primary care providers. These memberships ensure clients are never alone when trying to make difficult health and lifestyle changes. Your prevention, medical care and health information is seamlessly integrated with our intimate team.

Integrate your medicine, nutrition and fitness needs all through this one membership. Our physician and health coach work with you to develop personalized prevention goals and support your medical needs. Health coaching sessions focus on education and helping to support lifestyle changes through accountability. This program includes:

For clients under 30 we offer a discounted rate. This rate applies when there is an existing family member participating in any one of our other care membership.
This membership is sought after by clients interested in additional support from their physician. Your physician is able to come to you and is accessible virtually at no additional cost. It also includes office services such as travel and specialist coordination support. These services are not covered by insurance and come with an out-of-pocket annual fee.
As part of this membership you will receive email and mobile phone information for your doctor. There is no additional charge for phone, text or Email consultations as part of this membership.
In this membership your doctor is able to come to you. As needed, your physician will make visits both to short-term rehabilitation, skilled nursing and in the home.
This membership includes on-site Emergency Department consultations at the local hospitals. Your provider will help coordinate your care in person in the event of hospitalization.
As a member of this membership we waive our administrative fees. This includes: any requests for medical records, medical leave or other forms, cancellation fees, prior authorizations and phone consultations.
This membership provides services to help while you are away like our travel medical kit for emergencies and a personal medical flash drive, updated annually.
To help facilitate complex and busy life schedules, our office will also help schedule your appointments with specialists.
